A. FOR CHANGE OF STATUS TO F1(from any current nonimmigrant status, such as H-1B to F1)<o>

</o> 1. Copy of F1 applicants passport pages bearing photo and basic biographical information (name,
address, date/place of birth, expiration date of passport, place of issue of passport)
2. Copy of I-94 (very important)
3. Copy of I-20 (if applicant has been granted admission to a US school) ALL FOUR PAGES OF THE I-20<o>></o>>
4. Copy of current nonimmigrant visa from passport (H-1, etc.) and H1 approval notice and I-94;
5. How will you pay school tuition? Include latest bank statement, copy of tuition waiver or assistantship or scholarship award. Include your latest paystub from H1B employer.
